At Hunsaker Dental, we want your first visit to feel easy and stress-free.

After you book your appointment, you’ll receive a New Patient Email with a link to complete your paperwork online. This is the quickest option, and once you submit the forms, we’ll receive them right away. If you have any questions along the way, feel free to give us a call—we’re happy to help.

If you run into any trouble completing the forms online, no problem. You can download the PDFs below, print them out, and bring them with you to your appointment. If you’re using dental insurance, we kindly ask that you bring your completed paperwork at least two business days before your visit so you can have the experience with up-to-date information and no delays from your insurance company.

If you need a PDF viewer, Adobe Acrobat can be downloaded for free.

These forms are available in PDF format and cover important information such as your medical history, dental history, and contact details. By completing these forms ahead of time, you can save valuable time during your first visit and allow our team to focus on providing you with the best possible care.

We recommend that you bring your completed forms, referral slip (if applicable), a list of medications you are currently taking, and your insurance information card and photo ID to your initial appointment.
